Learning and trying out life skills is an important step for students as they plan to move into adulthood. For many students with special health care needs and/or disabilities, planning for that transition can include developing skills needed to manage their own healthcare.
The Parent Educational Advocacy Training Center (PEATC) and Got Transition have created a health care transition checklist to help students, families, and the IEP team plan for this specific area of transition to adulthood. Designing Value-Based Payment Innovations for Pediatric-to-Adult Transitional Care: Lessons from the Field | Wednesday, January 26 | 1:00 pm -2:00 pm (ET)
The National Alliance to Advance Adolescent Health will host a webinar to: 1) discuss a step-by-step approach to designing value-based payment (VBP) innovations for pediatric-to-adult transitional care, and 2) hear directly from senior officials in two large Medicaid MCOs – Florida Sunshine Health and Texas AmeriGroup – about their own experiences with designing a transition VBP pilot. This webinar is intended to assist state Medicaid agencies, commercial insurers, health plans, and pediatric and adult practices to expand their value-based efforts around pediatric-to-adult health care transition.
